What is Home Hospice?

Hospice is a comprehensive program that offers compassionate care and support to terminally ill people and their families at the end of life.

Our focus is on comfort and quality of life. Care is provided in a holistic approach, ensuring care needs are met physically, emotionally and spiritually. Care can take place anywhere a patient calls home – their own home, a nursing home, an assisted living or other long-term care facility. A multi-disciplinary approach to care strives to ensure patients are free of pain and symptoms, allowing them to live their last weeks and months to the fullest.

Hospice services include: care for the patient, grief support groups, volunteer services and other resources designed to help families and their loved ones throughout their hospice journey. Families and loved ones have a built-in support system with hospice.

If home hospice is not an option and a facility setting is more appropriate, Memorial also offers hospice at Heritage for patients who qualify.
