Calling Hospice

Anyone can make an inquiry to hospice – patients, family, loved ones, doctors, etc.

Upon contacting hospice, our team will answer your questions and address any concerns you may have. Many families choose to schedule a free no-obligation home consultation. This visit offers an opportunity for family discussion - we’ll explain services of the program, goals of care, and provide an understanding of what to expect as services begin. Hospice is often a very misunderstood topic and we welcome a group discussion to address any fears, concerns or misconceptions.

What to Expect

Once you’ve made the choice to elect hospice services, we’ll schedule your admission visit. Our admission team will make a home visit to complete required paperwork, fully explain services, perform a physical assessment, review medications you are taking and order any necessary equipment or supplies you may need. Our admission nurse will also coordinate with your physician to update him/her on the status of your care.

There is no one-size-fits all approach to hospice – the care you receive will be catered to you, and your family’s specific needs. Families are assigned a primary nurse who will visit your home to provide and coordinate care.

Initially, you may receive visits from the nurse and aide once or twice a week, while other team members may visit less frequently. The frequency of visits all depends on your specific needs.

Your plan of care and visit schedule will be ever-changing. As your condition changes and needs arise, your visit schedule will be adjusted as necessary.
