Memorial Home Services offers a telehealth monitoring program to assist with monitoring patients in the home. Telehealth involves an automated device placed in patients' homes to take their vital signs every day, remind them to take medication and answer simple questions about their condition in the comfort of their home.
How does Telehealth work?
Each day the Telehealth monitor will talk patients through checking vital signs including heart rate, blood pressure, oxygen levels and weight. The device can also ask questions pertaining to the patient's specific diagnosis.
In less than three minutes, health data is easily collected in the patient's home and securely transmitted to a nurse in the office to review and follow up with the patient as necessary.
Who qualifies for Telehealth?
Conditions that are considered for telehealth monitoring include:
- Congestive Heart Failure
- Chronic Obstructive Pulmonary Disease
- Cardiovascular Disease
How does Telehealth benefit patients?
With the Telehealth monitoring system, nurses can manage patient health seven days a week. Patients can have a sense of comfort and security knowing that their health is being monitored every day. Over time Telehealth decreases hospitalization and ER visits while improving clinical outcomes.
